Patient Rights And Responsibilities

As a patient of SMGH, you have the following rights:

• You have the right to be informed and understand your rights at the earliest possible moment. If you don't understand or need help, SMGH will provide assistance, including an interpreter.
• You have the right to considerate, respectful, dignified care from competent personnel, in accordance with your personal values and beliefs. SMGH will respond to requests for spiritual and/or religious observance.
• You have the right to be introduced to all healthcare personnel providing and coordinating your care. SMGH staff will visibly wear their ID badges at all times.
• You have the right to receive clear and complete information in layman's terms about your diagnosis, treatment, and prognosis, including pain management treatment, benefits, and risks.
• You have the right to consideration of your privacy concerning your medical care. This includes: upon request and if available, having a same-sex treating physician and/or nurse; having a person present during certain parts of a physical examination, treatment, or procedure performed by a health professional of the opposite sex, as long as this does not interfere with care provision; not remaining disrobed for longer than required for medical purposes; wearing appropriate personal clothing or religious or other symbolic items, as long as they do not interfere with diagnostic procedures or treatments; requesting a transfer to another room (if possible) if another patient or visitor is unreasonably disturbing you; requesting to be placed in protective privacy when considered necessary for personal safety.
• You have the right to formulate an Advance Directive. An Advance Directive is a legal document informing healthcare providers and loved ones about desired healthcare and decision-makers when you are unable to make decisions. SMGH will honor written Advance Directives, except when in conflict with professional standards or law. An Advance Directive is not a requirement for care at SMGH but is highly advised.
• You have the right to receive your medical records pertaining to your medical care and treatment confidentially.
